Apprentice Social Prescriber

A great opportunity has become available for a surgery in the S10 area of Sheffield. This is an ideal opening for someone who is interested in becoming a Social Prescriber or a Health and Wellbeing worker to support experienced Social Prescribers.

• Forge strong links with GP Practices, local voluntary and community organisations, utilising their established networks.
• Be a friendly source of information about wellbeing and prevention approaches.  
• Work with people, their families and carers and consider how they can all be supported.
• Support local partners and commissioners to develop new groups and services where needed.
• Build relationships with staff in GP practices within the local Primary Care Network (PCN), attending relevant meetings, becoming part of the wider network team, giving information and feedback on social prescribing.
• Work closely with GP practices within the PCN to ensure that social prescribing referral codes are inputted into the clinical system.
• Where appropriate, physically introduce people to community groups, activities and statutory services. Follow up to ensure they are happy, able to engage, included and receiving good support.
• Support the Social Prescribers produce a personalised support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services. 

As part of your apprenticeship, you will be required to attend college once per week at Sheffield College - City Campus.
